Adoption Ambassador and In Training Dog Adoption Application
Thank you for your interest in adopting from the Kentucky Humane Society! This form is to be used for dogs who are part of our Adoption Ambassador Program (meaning they are available for adoption through a foster home) or our In Training Program (meaning they are undergoing behavior training at KHS). An adoption counselor will review your application and will contact you as soon as possible to set up a phone interview. Please note that the dog you are interested in may already have pending applications, so submitting this form is not a guarantee that the animal is still available.
