Ready to Adopt? Streamline the Adoption Process by Completing an Online Adoption Interest Form!
We’re excited that you’re considering adopting a shelter animal from the Kentucky Humane Society. To make your adoption experience faster, please complete an online adoption interest form before you visit. The person who completes this form must be present at the time of adoption and have a valid ID. Here’s what you need to know:
- When you fill out the online adoption interest form before you arrive, you’ll save time while at our adoption sites!
- Your adoption interest form is valid for 6 months and applies to one animal adoption.
- It is important that you complete a new form if your information changes, if you want to adopt again, or if 6 months has passed since you last completed the form.
- Once you’ve completed the adoption interest form, come to our Sam Swope Pet TLC or East Campus to meet our adoptable cats and dogs.
- When you find your perfect match and decide to adopt, one of our adoption counselors can digitally search for your profile to expedite the adoption process.
IMPORTANT INFORMATION
This form is for those who are interested in adopting an animal from our Sam Swope Pet TLC and/or East Campus, not from Purrfect Day Cafe or Willow Hope Farm. If you are completing this interest form for an animal who is listed as “In Foster” or “In Training” a staff member will reach out to you via email within 72 hours if we feel that this pet could be a good match for your home. Completing this form doesn’t put a specific animal on hold or guarantee adoption. It simply helps us process your application more efficiently while you focus on finding your perfect match!

After completing your adoption interest form, come meet our adoptable dogs and cats. Walk-ins are always welcome at our Sam Swope Pet TLC (241 Steedly Drive) and East Campus (1000 Lyndon Lane). If you don’t meet the perfect pet for you during your first visit, please come back; we welcome new animals to KHS daily!
